Subject: Mandatory procedures under HIPAA

Hi Amit and all!

Compliance with HIPAA boils down to laying down procedures,
following those procedures, and documenting the processs.

1) Computer Security:

a) Physical access to computers and removable media (floppies, zip
disks, CD-ROM, etc.) must be controlled by means of locking office
doors, locking cabinets/drawers and access control cards and
security personnel.

b) User access to computers containing patient information must be
controlled by means of software or hardware based methods.
( Passwords/fingerprint scanning devices etc)

c) All backup containing patient information must be protected by
of encryption. Access to these files must be limited to individuals
directly involved in the transcription of patient information.
PGP gives you a choice of public/private key encryption:

d) A computer access logging program must be used to record
computer activity such as file accesses, etc. HIPAA requires us to
keep audit trials (to document when files containing patient
information are accessed and by whom). others available; Here's a
good place to start looking:

e) A firewall program must be utilized on all computers containing
patient information connectedt the net.

2) Hard copies / Audio recordings

a) All printed material containing patient information must be
secured by means of a locking container until such time as delivery
takes place.

b) All printed material containing patient information which is not
required to be kept for internal audit purposes must be destroyed
immediately. Databases like patient lists, which have to be saved
for future reference must be secured.

c) All analogue/digital tapes or other recording media containing
patient information must be secured in a locking container while
awaiting transcription. After transcribing, they must either be
erased immediately or secured.

d) All printed material and/or unerased tapes containing patient
information which is to be delivered to a client will be secured in
a locked vehicle and/or a locking container (briefcase, file
box,etc.) till delivery.

e) Access to printed or recorded patient information must be
limited to individuals on a need to know basis. A printed
log or other mechanism must be maintained in order to record all
accesses to printed or recorded patient information. A simple log
sheet with fields for date, time, initials and reason for access is
the minimum requirement.

3) Internet Security

All data which are transmitted over the Internet will be secured by
means of encryption. PGP works well for this purpose as well. Also
needed is access authentication.

4) Employee Confidentiality Agreements

a) All employees who are privy to patient information in any form
must be informed of all applicable regulations and company
protocols regarding patient information privacy, and  must sign a
confidentiality agreement stating that the employee has been
informed of such regulations and protocols, understands them, and
agrees to conform to them.

b) A written procedure must be instituted to provide for
appropriate steps to be taken in case of a breach of
confidentiality by an employee

5) Contracts

A written contract between the transcription service and each
client must be drafted and signed by all parties. The contract must
address privacy considerations for the patient information that
each client will share with the transcription service as a
"business associate".

Hi Dr. John and MTIDers!

If you have access to your prime source in the U.S. ask the point
of contact to offload the physicians list (i.e. Authors list data)
from the voice processing system, this file can be offloaded as
ASCII text and used as reference during transcription.  It should
cover all the physicians that you might encounter with that
particular account...

I did the same for our facilities and found helpful...

Subject: Demographics and Names in Medical Transcription

Hi Dr. John:

"Names" is not really a critical issue for most MT companies since
most of the clients do fax the list of Patient Schedules containing
patient appointment lists, Doctor/Dictator names, etc.

But when you don't get them, then what do you do? A good
transcriptionist, well conversant with American culture, can always
spell common names like John, Stephen, David, Joshua, Melissa,
Stephanie, etc. And in most cases, they are right too. It is the
last name which creates problems. In my experience of over 4 years
in this field, the client does not penalize you for writing
sound-alikes for names, or leaving the names as blanks. Some
clients do get finicky about them; in such a case, take the help of
Internet. I would suggest you take the help of a search engine,
"google" being the best for finding out last names. You can also go
through the following link for most popular names of 1990s:

As regards doctor names, again take the help of Internet for
finding out a list of doctors of the area where your client is
from. In most cases, you would find at least some doctor listing on
some sites. Again, I would suggest a site for the same:

But one thing that takes precedence over everything else is don't
write the name when you are not sure. I think that is what really
bugs the clients.

Subject: CMT exam

I feel that any who are serious should consider the certification
exam for MTs. It is unfortunate that the exam currently is not
being given in India. I have had contact with Lynda Byrne of MTCP,
the testing agency for certification for AAMT regarding this
question, and she has mentioned that there is about to be a change
in the way this exam is given. It would certainly be appropriate to
check on that and also to contact MTCP through www.aamt.org for any
questions regarding new testing procedures and/or the possibility
of a new organization giving the exam.

Subject: CMT certification in India

Mr Subhorup Dasgupta, Amit and Friends at MTID:

I am a medical professional (consultant and teacher in Respiratory
Medicine) who has done an 8 months (part-time) course in MT.

I am echoing the sentiments of Mr Dasgupta. There is no control or
sanity whatsoever in the field of MT training in India as on date.
Any body (smart enough!) can start a training institute for
prospective transcriptionists. With so much of talent in this
country, is it too wishful to think that we, those Indians
genuinely interested in MT, could start a set up to
train/assess/certify/grade those who have (or purports to have)
exposure to MT? If something is not done immediately in this
direction, the credibility of all of us associated with MT is at
stake.

I would like to see a facility where any body interested in MT
could learn the ropes, free of cost on the internet. Any takers for
this idea?

Dear Amit,

I follow all the posts and reviews of your digest. I am at present
a housewife having given up my full-time job in a bank. I want to
work from home and have experience of doing data-processing jobs
from home. I have completed my six months MT course from
IMT( which incidentally has closed down). I would like to know if I
can work from home and whether there are instituions outsourcing
jobs to individuals. Please help me out as I want to put to use the
training I have undergone and also earn some money. Also what is
the salary a fresh MT should expect? I have heard a lot of
unsavoury stories of exploitations in this regard.
